As the vehicle owner and policyholder, I confirm that:

• I hold a full driving licence, am over the age of 18 years, and am a resident of the UK, or I am a serving member of HM Armed Forces.
• I have purchased my vehicle within the last 180 days. It is under 8 years old, with less than 80,000 miles travelled and is not an ineligible vehicle.
• I am the comprehensive motor insurance policyholder for this vehicle, or I am listed as a named driver and do not already have Gap cover for this vehicle.
• My vehicle is not used for hire & reward, or for short term rental (this does not apply to taxis, private hire or driving school vehicles).
• I have read and accept the Policy Terms & Conditions, Privacy Policy and significant exclusions.
• I have read, understood and accept the information disclosed to me in the Insurance Disclosure Document.

The answers I have given are true and complete, and I have declared all material facts that may affect your decision to accept the vehicle for cover. I understand if I have not given full and true answers this could affect any claim I make.
